Fictitious Business Name Statement

Mail the completed Fictitious Business Name Statement to THE SACRAMENTO GAZETTE at PO Box 816, Sacramento, CA 95812-0816 with your two checks. (Be sure the Fictitious Business Name Statement has an original signature.)

Once your completed statement is received, we will hand-deliver your statement to the Sacramento County Clerk-Department of Finance for filing. Once the filing is completed, we will mail you a filed copy for your records.

After the four-time publication is completed, we will personally deliver your proof-of-publication to the Sacramento County Clerk-Department of Finance. For your convenience, we will also mail you a duplicate copy endorsed/stamped by the Sacramento County Clerk.

• Check #1: "The Sacramento Gazette" for $114.95, plus $5 for each business name and/or owner over one;
  
• Check #2: "Sacramento County Clerk" for $44, plus $8 for each business name and/or owner over one. Please mail both checks together with your completed form to THE SACRAMENTO GAZETTE.

Mail one copy of your filed Fictitious Business Name Statement with your check to THE SACRAMENTO GAZETTE at PO Box 816, Sacramento, CA 95812-0816.
